Since there is hardly any documentation available on how to accomplish this, I would like to share a classic tutorial on how to generate a customized activity in Adobe Campaign Classic. This tutorial is available in both English and French (I will provide the French version upon request), and I hope that the instructions included will provide enough details to help you develop personalized activities with your own context.

@krishnanunni its not possible to change the form structure. You may be able to trigger scripts on form inputs.

Thank you for sharing the document. I would like to know whether any of the below two cases are possible. If any of them is possible, please share a brief guidance on how to achieve it. Thanks!
1. Render the form based on an external API response. This means I would like to show some dropdowns or a table, listing some data that I get from an external API.
2. Render an external webpage inside custom activity.
