How to pre-fill recipient information into WebApp/Survey

Hello Guys, 

my intention is to make an WebApp form, where the recipient just klicks the link in the E-Mail and the form open's up. All informations related to the recipient are already pre-filled so they just have to accept the offer.

Do I have to make a SurveyID which is connected to the Recipient-Database or is there a simpler way?
