AEM 6 Form | show lightbox on radio button option selection

Hi Guys, 

I am working a task to create a form in AEM 6.0 (Adaptive forms) and need to show a lightbox pop with some text on it on selecting one of the option in a radio group.

This has to be done for two separate radio button groups and each lightbox has different text in it.

Has anyone tried something similar or know how can we achieve this?
