Change in triggers or user error?

I have an issue where if a new person is created during a form fill, it is not triggering "Fills Out Form" for campaigns containing that form.

For example:
- A net-new person is created when webinar registration form is filled out
- Marketo captures the person, starts scoring, etc., but doesn't place them into this campaign:
