Email Preference Center Functionality

We are looking to set-up the following functionality in Marketo for a Preference Center form. We currently have 4 checkboxes on the form - 3 checkboxes are types of communications to Opt-In to receive, and 1 checkbox is an unsubscribe. If unsubscribe is selected, the other 3 checkboxes should be false. If 1 or more of the 3 opt-in checkboxes is selected, the unsubscribe should be false. Looking for assistance on implementing this functionality. Thank you!

There are dozens of threads and articles on this. Please search.

You will need some basic javascript to do the on page changes. Suggest a developer or checking to see if someone posted code here.

Josh - I tried implementing the code here but it does not seem to be working: Email Preferences | Discovery Data  Would you be able to advise what the issue may be?
