Form as dynamic content in a Marketo landing page

@sanfordwhiteman I am not able to add the form as a dynamic element inside a Marketo landing page. For instance, I want to show a separate form for a different segments of the audience. I created the segment, tried to add separate forms under each segment under the dynamic settings of the landing page. Somehow, the green tick is not clickable, and the result is not dynamic as well.

Can you please help here?
