Form In Landing Page Showing Up Twice

Can someone help explain why a form on a landing page would be showing up twice? Not sure how I can fix this. Need some assistance! See here and below: info.startmoresmiles.com/PrepareForSameDayStarts.html

I see the form code on your page twice as well. Is this a marketo landing page, or CMS?

The code I see is on line 306 & 423

Because you have the same form twice in the page. Marketo will stack the forms within the same container unless you take special measures as I described here: https://blog.teknkl.com/multiple-forms-multiple-times/
