How do I create a leads report with all fields data

I have created this report, but I need to add additional fields to the RHS, basically, I need a report:

1) who signed up today

2) all the data they entered into a specific form

3) emailed to me and my colleagues

4) downloadable into Excel.

Sorry must a Friday morning thing, but I just can't see how to do this.

The answer after much messing about in reports.

The solution I found is to create a Smart List,
Create a customised View of this smart list - adding in all the custom fields from my form
Subscribe or export the smart list
