Improve Forward to Friend Form

The  forward to a friend form is a great feature, but I think there is a lot of opportunity here for improvement.

1. A customizable form would be ideal so that companies can create a form that is on brand and personalized. I think it would be great to be able to:

edit the title, add copy, and add additional fields when appropriate.

2. Ability to send a personalized message to the person they are forwarding the email to. This way it's clear who forwarded the email and why they think

the piece of content is relevant to the recipient.
