Marketo Form on Wistia Video- Thank You Experience

We're trying to add a Marketo form at the end of our product videos that are hosted in Wistia. I set up a Marketo page and embedded the video. In Wistia, I added the form at the 1 minute mark of the video. When you submit the form for the first time- the video just says "Rewatch"- I can't figure out how to add a thank you message after the form submission. 

In addition to this, when you go back to the video and watch it to the one minute mark, the form no longer shows- it just says "Rewatch" at the end of the video. I made sure that pre-fill is disabled, however I also read that doesn't matter. 

Our plan is to use the same form across 4 different videos that are hosted on 4 separate pages on our site (we have Marketo tracking code enabled).
