Please let me know how to include text box in the marketo form.

I'm assuming that you want to add a text box for adding help text/info/etc. in the form (and not the text box as field type where visitors can fill their data).

 

You can add a Rich Text field to the form, and add your text in it. Here's the link to Add Rich Text to a Form in the product documentation.
