Rachel,

You can read a bit more about it here: Alert the Sales Rep - Marketo Docs - Product Docs

You will need to create an email that includes all of the form's fields as tokens. You will then create a smart campaign where the trigger is "fills out form" and the flow step is "send alert". Include your email address in the "To Other Emails" field of the alert and nullify the "Send To" field.

Thank you for your response. If we want to get the information sent with the alert, how would that be setup? Or where does the information they fill out in the form go to? We need that information sent to me in an email once someone has completed the form.

This is one of those "indicator" questions...seems like you're pretty new to Marketo (which is awesome)...definitely spend some time on http://docs.marketo.com/ getting up to speed

Quick and easy: Use the Send Alert Info Token {{SP_Send_Alert_Info}} - Marketo Docs - Product Docs

More detailed: Learn about tokens and inserting them into emails.
