Tag prospects with an unsubscribed reason

I'm looking to indicate the tracks with the various unsubscribed reasons, such as for example:

 

a)Once a week sounds about right

b)Let's slow it down-only once a month, please

c)I take it back! Keep the inspiration coming.

d)It's just not working out. Unsubscribe me.

 

If  the lead choose the reason "a" it should flag & these leads are must filtered for the future campaigns, if the future campaign is before 1 week. In the same way the all options should work respectively

 

Can anyone help me out from this?
