Those who filled out form are not being recognized

Hi, what could be the reason why those records who filled out the form are not being added to the list nor recognized in the reports? However if you will check the record, you can see action in the activity log?

@lodiavlair_tipo sounds like the trigger may not be firing. 

Do you have a screenshot of your trigger that you can show?
