Tracking UTM in Wordpress

Hello all,

I am using Marketo form in multiple places on my Wordpress website and I am unable to see UTM parameters passing through to Marketo when someone fills out a form.

The form looks like this 

The auto-fill settings for those fields are set to take the parameters from the URL per other suggestions I have seen but I cannot see utm information passing through from Wordpress into Marketo.

I am copying and pasting the form into a page builder (Elementor) and it renders well, and it seems to have all the other characteristics of the form, I'm not sure what I am missing to pass the utm parameters through.
