Generate an Approval Audit Trail and Create/Upload Document to the Task

I am trying to build a scenario that will collect stage approvals from a task approval process after final stage approval, create a document with the stage approval, and then upload to the task.

Scenario setup:

I am watching updated tasks to see when Task approval workflows are complete. From there I am collecting more task data, owner data, and assignee data. Then I create a record to update to add a note and change the status from "In Progress" to "Approved". I'd like to capture the approvers from each stage and the start/completion dates for an audit report that would be created/uploaded to the task.

Has anybody created a similar solution?
