how to upload a new custom DOR

I see that the templates to create a document of record are under

I installed AEM forms designer and created a Custom DOR for my needs now I just need to know how to place it in here so that I can use it with my new forms?

Upload it anywhere on the forms portal(where you see all other adaptive forms) using the file upload option of forms portal.

Depending on where you upload you can search any xdp

you can map the DOR from the form properties. Just below the form data model.
