Is there Javascript code I can embed to Reader Extend forms to allow for attachments to be added?

no, you cannot apply reader extensions with JavaScript from inside the PDF. For Attachments this is a modification of the PDF by AEM Forms server.

@shandic You can reader extend via acrobat or via server side code.
